TITLE
Recommendation to authorize City Attorney to submit two claim for death benefits for approval by the Workers’ Compensation Appeals Board, and if so approved, authority to pay 1) $274,692 for Stipulations with Request for Award, and also 2) separate authority to pay $20,000 for Compromise and Release in compliance with the Appeals Board Order for decedent, Stevaughn Matthews.
DISCUSSION
Stevaughn Matthews, by and through Jonelle Cummings as Guardian ad Litem for his daughter, Kailey Matthews, and dependents, Jonelle Cummings and Sharon Matthews have made a claim for death benefits allegedly caused by his employment with the City of Long Beach. The Economic Development and Finance Committee on November 16, 2021, approved the submission to the Workers’ Compensation Appeals Board of 1) a Stipulations with Request for Award for Death Benefits to Jonelle Cummings as Guardian ad Litem for Kailey Matthews, in the amount of $274,692.00, and 2) a Compromise and Release for death benefits in the amount of $20,000.00 with $15,000.00 of the amount paid to Jonelle Cummings and $5,000.00 to Sharon Matthews by the City and Stevaughn Matthews on behalf of his dependents.
