Recommendation to authorize City Manager, or designee, to execute an agreement and any subsequent amendments between the City of Long Beach and Kaiser Foundation Hospitals to accept funding in the amount of $60,000 for the period of July 1, 2014 through June 30, 2017 for community outreach, education, screening, and referrals for diabetes and prediabetes. DISCUSSION
Diabetes is a chronic disease that can cause serious health complications, including blindness, kidney failure, and heart disease. According to the 2011 Los Angeles County Health Survey, an estimated 12.2 percent of adults in Long Beach have been diagnosed with diabetes. Diabetes can be prevented or managed through education, healthy eating, and physical activity.
The Kaiser Foundation Hospitals has awarded the City of Long Beach $60,000 for the Department of Health and Human Services’ (Health Department) Diabetes Program to enhance community outreach, education, screening, and referrals for diabetes and prediabetes.
The Diabetes Program is an expansion of the Health Department’s Latino Diabetes Prevention and Management Program, which was established in September 2003. The program consists of a multi-week diabetes education course, available in English and Spanish, that focuses on self-management, skill building, and social support. The program is provided at no cost and is open to the public.

TIMING CONSIDERATIONS
City Council action on this item is requested on November 11, 2014, in order to accept funding and begin program operations to meet required objectives.
FISCAL IMPACT
The total amount of the agreement is $60,000 over three years. Sufficient funds are budgeted in the Health Fund (SR 130) in the Department of Health and Human Services (HE). There is no City match of funds required, and no local job impact associated...
