TITLE
Recommendation to authorize City Attorney to submit Stipulations with Request for Award and Compromise and Release for approval by the Workers' Compensation Appeals Board, and if so approved, authority to pay (1) $59,542.50 for Stipulations with Request for Award (2) $2,500 for Compromise and Release; and (3) $5,000 to settle Vocational Rehabilitation (for a total amount of $67,042.50) for claim of William Wallace in compliance with the Appeals Board Order.
DISCUSSION
William Wallace has made a claim for injuries allegedly sustained in the course of his employment with the City of Long Beach. The Economic Development and Finance Committee on November 1, 2005, approved the submission to the Workers' Compensation Appeals Board of Stipulations with Request for Award by the City and William Wallace, based upon a 60 Y2 percent disability rating, with provision for lifetime medical care as may be necessary, a Compromise and Release agreement, and a buy-out of vocational rehabilitation.
