TITLE
Recommendation to authorize City Attorney to submit Stipulations with Request for Award for approval by the Workers' Compensation Appeals Board, and if so approved, authority to pay $33,320, and $10,766.25 for a total of $44,086.25 for claim of Debra Bonesteel-Smith in compliance with the Appeals Board Order.
DISCUSSION
Debra Bonesteel-Smith has made a claim for injuries allegedly sustained in the course of her employment with the City of Long Beach. The Economic Development and Finance Committee on October 4, 2005, approved the submission to the Workers' Compensation Appeals Board of stipulations with Request for Award by the City and Debra Bonesteel-Smith, based upon a 40 percent disability rating, and a 17 percent disability rating, both with provision for lifetime medical care as may be necessary.
