TITLE
Recommendation to authorize City Attorney to submit Stipulations with Request for Award for approval by the Workers' Compensation Appeals Board, and if so approved, authority to pay (1) $194,737.50 for Stipulations with Request for Award, and also (2) to pay $216.46 per week for a Life Pension thereafter, subject to cost of living adjustments in compliance with the Appeals Board Order for Lessa Mahaffey.
DISCUSSION
Leesa Mahaffey has made a claim for injuries allegedly sustained in the course of her employment with the City of Long Beach. The Economic Development and Finance Committee on March 5, 2013, approved the submission to the Workers' Compensation Appeals Board of Stipulations with Request for Award by the City and Leesa Mahaffey, based upon (1) an 88 percent disability rating and (2) a further provision for a Life Pension at the rate of $216.46 per week, subject to cost of living adjustments, after the permanent disability has been fully paid. Finally, there would be a provision for lifetime medical care as may be necessary.
