Recommendation to authorize City Attorney to submit Stipulations with Requests for Awards for approval by the Workers' Compensation Appeals Board, and if so approved, authority to pay, in compliance with the Appeals Board Order(s), the following amounts: $17,713.75 and
$17,713.75 (for a total amount of $35,427.50) for the claim of Kelvin Blackburn; $53,422.50, $38,080, and $1,260 (for a total amount of
$92,762.50) for the claim of Mark Cooper; $27,370 for the claim of Richard Holmes; $120,865 and $29,750 (for a total amount of
$150,615) for the claim of James Manning; and $40,460 and $10,000 to settle Vocational Rehabilitation Benefits (for a total of $50,460) for the claim of Sherrie Paige.
DISCUSSION
Kelvin Blackburn has made a claim for injuries allegedly sustained in the course
of his employment with the City of Long Beach. The Economic Development
and Finance Committee on September 20, 2005, approved the submission to the
Workers' Compensation Appeals Board of Stipulations with Request for Award
by the City and Kelvin Blackbum, based upon a 1) 25% and 2) 25% percent
disability rating, with provision for lifetime medical care as may be necessary.
