TITLE
Recommendation to authorize City Attorney to submit Stipulations with Requests for Awards for approval by the Workers' Compensation Appeals Board, and if so approved, authority to pay 1) $4,235 and 2) $18,050 for the claim of Lillian Armenta; and $7,200 for claim to Alfonso Esqueda in compliance with the Appeals Board Orders.
DISCUSSION
Lillian Annenta has made a claim for injuries allegedly sustained in the course of her employment with the City of Long Beach. The Economic Development and Finance Committee on May 2, 2006, approved the submission to the Workers' Compensation Appeals Board of the two Stipulations with Request for Award by the City and Lillian Annenta, based upon a 10 percent disability rating, and a 20 percent permanent disability, both with provision for lifetime medical care as may be necessary.
Alfonso Esqueda has made a claim for injuries allegedly sustained in the course of his employment with the City of Long Beach. The Economic Development and Finance Committee on May 2, 2006, approved the submission to the Workers' Compensation Appeals Board of Stipulations with Request for Award by the City and Alfonso Esqueda, based upon a 9 percent disability rating, with provision for lifetime medical care as may be necessary.
