DISCUSSION
Since 1997, the City of Long Beach Department of Health and Human Services (Health Department), has been providing Targeted Case Management (TCM) services to both eligible Medi-Cal and non-Medi-Cal beneficiaries under agreement with the State of California Department of Health Care Services (State).

On June 11, 2013, the City Council approved the current TCM agreement with the State ending June 30, 2018. Under this agreement, Health Department staff assist eligible clients with significant health needs in accessing critical medical, social, educational and other services.

This year, over 600 individuals received case management services through TCM from the Health Department. The beneficiaries being mostly children and medically fragile individuals.

The State is requiring the Health Department to enter into a Memorandum of Understanding (MOU) with Health Net, a Managed Care Health Plan provider for Medi-Cal beneficiaries in Los Angeles County. The purpose of the MOU is to ensure there is no duplication of services between the Health Department TCM program and the medical case management services provided by Health Net. The term of this MOU extends beyond the Department’s current TCM agreement, as the State typically enters into a MOU for a five-year period. However, the MOU may be terminated by either party, at any time, by providing a 30-day written notice. Furthermore, since State TCM funding is anticipated to continue, this MOU will automatically renew on a yearly basis without action by either party, unless terminated earlier as described above.
