TITLE

Recommendation to authorize City Attorney to submit four Stipulations with Request for Award for approval by the Workers’ Compensation Appeals Board, and if so approved, authority to pay 1) $111,142.50 and 2) separate authority to pay $26,245 and 3) separate authority to pay zero percent for Stipulated Award and 4) separate authority to pay zero percent for Stipulated Award in compliance with the Appeals Board Order for Jeffrey Jonkey.

 

DISCUSSION

Jeffrey Jonkey has made a claim for injuries allegedly sustained in the course of his employment with the City of Long Beach.  The Government and Personnel Attorney-Client Committee on March 15, 2022, approved the submission to the Workers’ Compensation Appeals Board of four Stipulations with Request for Award by the City and Jeffrey Jonkey, based upon 1) a 64 percent disability rating, with provision for lifetime medical care as may be necessary, and 2) a 23 percent disability rating, with provision for lifetime medical care as may be necessary, and 3) a zero percent disability rating, with provision for lifetime medical care as may be necessary, and 4) a zero percent disability rating, with provision for lifetime medical care as may be necessary.

Approve recommendation.
