DISCUSSION

Ronald Hernandez has made a claim for injuries allegedly sustained in the course of his employment with the City of Long Beach.  The Economic Development and Finance Committee on May 6, 2008 approved the submission to the Workers’ Compensation Appeals Board of two Stipulations with Request for Award by the City and Ronald Hernandez, based upon 1) a 34 percent disability rating, with provision for lifetime medical care as may be necessary, and 2) a 28 percent disability rating, with provision for lifetime medical care as may be necessary.

 

Thomas Jaramillo has made a claim for injuries allegedly sustained in the course of his employment with the City of Long Beach.  The Economic Development and Finance Committee on May 6, 2008 approved the submission to the Workers’ Compensation Appeals Board of Stipulations with Request for Award by the City and Thomas Jaramillo, based upon a 55 percent disability rating, with provision for lifetime medical care as may be necessary.

 

Gregory Keyes has made a claim for injuries allegedly sustained in the course of his employment with the City of Long Beach.  The Economic Development and Finance Committee on May 6, 2008, approved the submission to the Workers’ Compensation Appeals Board of Stipulations with Request for Award by the City and Gregory Keyes, based upon a 48 percent disability rating, with provision for lifetime medical care as may be necessary.

 

John Lee has made a claim for injuries allegedly sustained in the course of his employment with the City of Long Beach.  The Economic Development and Finance Committee on May 6, 2008, approved the submission to the Workers’ Compensation Appeals Board of three Stipulations with Request for award by the City and John Lee, based upon 1) a 78 percent disability rating with further provision for a life pension at the rate of $139.15 per week after the permanent disability has been paid, with provision for lifetime medical care as may be necessary, and 2) an 8 percent disability rating with provision for lifetime medical care as may be necessary, and 3) a 13 percent disability rating with provision for lifetime medical care as may be necessary.
