TITLE

DISCUSSION
The California Endowment (TCE) has awarded the City of Long Beach $265,446 to implement the Long Beach Diabetes Collaborative Program and conduct an 18-month planning process to address diabetes. Under the agreement, the Department of Health and Human Services (DHHS) will be the lead agency in the Long Beach Diabetes Collaborative. The purpose of this collaborative project is to support a community-based effort to strengthen diabetes services and to address systems of care and support for diabetes prevention and management for low-income persons in Long Beach with or at risk of diabetes. Long Beach is one of two communities in the region to be selected by TCE, and its affiliated LA Health Action Office, to implement a geographic demonstration project to address chronic disease ainong adults.
 
The DHHS will work with local service providers, community-based agencies, faith-based organizations, and other community stakeholders to identify gaps in the system of diabetes care, create linkages among providers and resources, complete a diabetes needs assessment, pilot small-scale diabetes support interventions and to ultimately develop an action plan to create an improved system of diabetes care in Long Beach. The planning process will entail: convening monthly collaborative meetings; conducting community assessment meetings and surveys of providers and consumers; producing and disseminating a resource guide; training selected community agencies on diabetes selfmanagement; and, piloting self-management support programs in a clinical and community setting.
 
Of the total grant amount $37,227 is dedicated to subcontracts with community agencies that will conduct assessments, outreach, and piloting self-management support.  The following community-based agencies were selected based on their current diabetes prevention and management activities, their current work with The California Endowment, and their ability to reach special population groups:

TIMING CONSIDERATIONS
City Council action on this item is requested on April 4, 2006, as services under the agreement commenced on March 1 , 2006.
 
FISCAL IMPACT
The City will receive grant revenues in the amount of $265,446 to be expended from March 1, 2006 to August 31, 2007 These grant funds will be used to support the above subcontracts, and program staff, which may include 1.0 FTE Public Health Associate 111.  An appropriations increase is included in the recommended action.
