DISCUSSION

David Faris has made a claim for injuries allegedly sustained in the course of his employment with the City of Long Beach. The Government and Personnel Attorney-Client Committee Meeting on August 17, 2021, approved the submission to the Workers' Compensation Appeals Board of four Stipulations with Request for Award by the City and David Faris, based upon 1) an 11 percent disability rating, with provision for lifetime medical care as may be necessary, and 2) a 26 percent disability rating with provision for lifetime medical care as may be necessary, and 3) a 17 percent disability rating with provision for lifetime medical care as may be necessary, and 4) a 74 percent disability rating with further provision for a lifetime pension at the rate of $100.50 per week subject to COLA increases after the permanent disability has been paid, with provision for lifetime medical care as may be necessary.

Approve recommendation.
