TITLE
Recommendation to authorize City Attorney to submit Stipulations with Request for Award for approval by the Workers' Compensation Appeals Board, and if so approved, authority to pay (1) $160,177.50 for Stipulations with Request for Award, and (2) $154.61 per week for cost-of-living adjustments, for a Life Pension thereafter for Dale Mukanos, in compliance with the Appeals Board Order.
DISCUSSION
Dale Mukanos has made a claim for injuries allegedly sustained in the course of his employment with the City of Long Beach. The Economic Development and Finance Committee on June 7, 2011, approved the submission to the Workers' Compensation Appeals Board of Stipulations with Request for Award by the City and Dale Mukanos, based upon (1) an 80 percent disability rating and (2) a further provision for a Life Pension at the rate of $154.61 per week, subject to cost-of-living adjustments, after the permanent disability has been fully paid. Finally, there would be a provision for lifetime medical care as may be necessary.
SUGGESTED ACTION
Approve recommendation.
