TITLE
Recommendation to authorize City Attorney to submit Stipulations with Request for Award for approval by the Workers' Compensation Appeals Board, and if so approved, authority to pay 1) $3,450 and also separate authority to pay 2) $55,890 for the claim of Zade Michaels in compliance with the Appeals Board Order.
DISCUSSION
Zade Michaels has made a claim for injuries allegedly sustained in the course of his employment with the City of Long Beach. The Economic Development and Finance Committee on February 2, 2010, approved the submission to the Workers' Compensation Appeals Board of two Stipulations with Request for Award by the City and Zade Michaels, based upon 1) a 5 percent disability rating, with provision for lifetime medical care as may be necessary, and 2) a 46 percent disability rating, with provision for lifetime medical care as may be necessary.
SUGGESTED ACTION
Approve recommendation.
