DISCUSSION
Since 1998, Los Angeles County has contracted with the City of Long Beach Department of Health and Human Services (Health Department) to provide Human Immunodeficiency Virus (HIV)/Acquired Immune Deficiency Syndrome (AIDS) clinical services that include: medical outpatient, case management, and treatment advocacy programs.
 
The medical outpatient program provides medical services to approximately 300 patients annually and strives to interrupt or delay the progression of HIV/AIDS disease and promoting optimal health for the client. Services include: diagnosis, treatment, counseling, and referrals for appropriate specialty services.
 
Case management services include an individual client assessment to ascertain their physical, psychosocial, environmental and financial needs, and to facilitate their access to the appropriate service provider. The program serves approximately 250 individuals with HIV/AIDS infection who are medically indigent and would have no other means of accessing care. The Health Department also actively collaborates with and obtains referrals from service organizations and agencies beyond traditional HIV/AIDS case management service delivery systems, including but not limited to: substance abuse, mental health, specialty services, and social service organizations.
 
Treatment advocacy services are provided to every patient in one-on-one counseling sessions. These sessions include education on decreasing the risk of acquiring opportunistic infection, and on prevention methods aimed at reducing the risk of transmission of HIV to others. The program also provides the client with information on medications and their side effects and assists them in identifying major obstacles in treatment that could impact their health.
 
This matter was reviewed by Deputy City Attorney Linda Trang on January 4, 2010 and by Budget and Performance Management Bureau Manager David Wodynski on December 30,2009.
 
TIMING CONSIDERATIONS
City Council action is requested on January 19, 2010, as the Health Department has been providing services under the amendments since June 1, 2009 but will be unable to recover costs until the amendments are approved. Implementation of these agreements was delayed by the funding agency.
 
FISCAL IMPACT
The proposed amendments to the agreements with Los Angeles County include: case management services for an amount not to exceed $447,093, medical outpatient services for an amount not to exceed $182,000, and treatment advocacy services for an amount not to exceed $92,813. The total amount of the amendments is $721,906 for the period of June 1, 2009 through February 28, 2011. Sufficient appropriations currently exist in the Health Fund (SR 130) in the Department of Health and Human Services (HE) for FY 10. Funding for FY 11 will be appropriated as part of the FY 11 budget process. There is no impact to the General Fund.
