DISCUSSION
Since 1998, Los Angeles County has contracted with the City of Long Beach Department of Health and Human Services (Health Department) to provide Human Immunodeficiency Virus (HIV)/Acquired Immune Deficiency Syndrome (AIDS) clinical services that include: medical outpatient, case management, and treatment advocacy programs.
 
The medical outpatient program provides medical services to approximately 300 patients annually and strives to interrupt or delay the progression of HIV/AIDS disease and promoting optimal health for the client. Services include: diagnosis, treatment, counseling, and referrals for appropriate specialty services.
 
Case management services include an individual client assessment to ascertain their physical, psychosocial, environmental and financial needs and to facilitate their access to the appropriate service provider. The program serves approximately 250 individuals with HIV/AIDS infection who are medically indigent and would have no other means of accessing care. The Health Department also actively collaborates with and obtains referrals from service organizations and agencies beyond traditional HIV/AIDS case management service delivery systems, including but not limited to: substance abuse, mental health, specialty services, and social service organizations.
 
Treatment advocacy services are provided to every patient in one-on-one counseling sessions. These sessions include education on decreasing the risk of acquiring opportunistic infection, and on prevention methods aimed at reducing the risk of transmission of HIV to others. The program also provides the client with information on medications and their side effects and assists them in identifying major obstacles in treatment that could impact their health.
 
The proposed amendments to the agreements with Los Angeles County include: case management services for an amount not to exceed $54,232 for the period from March 1, 2009 to May 31, 2009, medical outpatient services for an amount not to exceed $25,077 for the period from March 1, 2009 to May '31, 2009, and treatment advocacy services for an amount not to exceed $8,839 for the period from April 1, 2009 to May 31, 2009. These amendments have extended the agreements through May 2009, however, due to the State budget crisis, the extensions for these services from June 2009 to date have been delayed. We will request City Council approval on the current term as soon as these new amendments are available.
 
This matter was reviewed by Deputy City Attorney Linda Trang on September 14, 2009 and by Budget and Performance Management Bureau Manager David Wodynski on September 22,2009.
 
TIMING CONSIDERATIONS
City Council action is requested on October 13, 2009 as the Health Department has been providing services under the amendments since March 1, 2009, but will be unable to recover costs until the amendments are approved. Implementation of these agreements have been delayed by the funding agency.
 
FISCAL IMPACT
The total amount of the agreements is $88,148 for the period of March 1, 2009 through May 31, 2009. Sufficient appropriations exist in the Health Fund (SR 130) and the Department of Health and Human Services (HE) to support this program. There is no impact to the General Fund.
