TITLE
Recommendation to authorize City Manager, or designee, to execute an agreement, and any subsequent amendments, with Community Partners, to accept and expend grant funding in the amount of $200,000, to participate in the California Accountable Communities for Health Initiative, for the period of September 1, 2019 through August 31, 2021, with the option to extend the agreement for two additional two-year periods, at the discretion of the City Manager; and
Increase appropriations in the Health Fund Group in the Health and Human Services Department by $200,000, offset by grant revenues. (Citywide)
DISCUSSION
Community Partners was established in 1992 to offer expert guidance, essential services, and support to grow creative solutions to community challenges. Community Partners has awarded the City of Long Beach a $200,000 capacity building grant for an initial period of two years. The grant is intended to support the Health and Human Services Department’s (Health Department) participation in the California Accountable Communities for Health Initiative (CACHI).
Through CACHI, the Health Department will plan and implement a model system that links health care systems, providers and health plans to public health, and community and social services organizations serving diverse communities across California. Grounded in an innovative vision to reform the health care system so that it is more responsive to people’s needs and environments, this model system would set a precedent and pave the way for developing health care systems that are more expansive, connected, and prevention oriented.
Since June 1, 2017, the Health Department has used CACHI funds to develop an Accountable Community of Health (ACH) for children.
