TITLE

Recommendation to authorize City Attorney to submit claim for death benefits for approval by the Workers’ Compensation Appeals Board, and if so approved, authority to pay death benefits to decedent’s minor children of $1,103.29 per week until July 13, 2024 when youngest minor child turns 18 in compliance with the Appeals Board Order for decedent, Brian Collette.  The weekly benefit is subject to State Average Weekly Wage Increases.

 

DISCUSSION

Brian Collette by and through Renee Collette, Guardian ad Litem for decedent’s two minor children made a claim for death benefits caused by his employment with the City of Long Beach.  The Economic Development and Finance Committee on December 15, 2015, approved the submission to the Workers’ Compensation Appeals Board of Stipulations with Request for Award for death benefits by the City and Brian Collette (deceased) on behalf of the minor children, Kyle (date of birth September 19, 1997) and Madison (date of birth July 13, 2006).
