Recommendation to authorize City Manager, or designee, to execute an agreement, and any subsequent amendments, between the City of Long Beach and the Archstone Foundation to accept funding in the amount of $10,120, for the period of July 1, 2015 through December 31, 2015, to be used for the Case Management Summit - Linkage to Services for Older Adults in Long Beach. DISCUSSION
Twenty-five percent (25%) of the City’s population, approximately 119,000 individuals, per the 2010 U.S. Census, are adults ages 50 and over. Nine percent (9%) are 65 years of age and over, with a median age of 73.5. Of those 65 years and over, 47 percent live alone, 13 percent live below the poverty level, 40 percent live with one disability, and 35 percent speak a language other than English. As the aging population grows in Long Beach, there is an increased need to explore avenues to improve linkages to service by addressing the multiple factors impacting accessibility.
To understand the landscape of services and referral networks available to the older population in Long Beach, the Archstone Foundation will sponsor the Long Beach Department of Health and Human Services’ Case Management Summit - Linkage to Services for Older Adults in Long Beach. The purpose of the summit is to bring together Long Beach providers who assist older adults in order to begin formulating an integrated health and social services linkage to care process.
The summit will be a half-day meeting held in Long Beach. Participants will discuss and share information that will be used for a gap analysis via mapping of service access points, transportation links, and availability of services for older adults in Long Beach. As part of the Health Department’s strategic plan goal of improving the health, function, and quality of life of older adults, the summit is the beginning of building a cross-agency older adult service network.
