TITLE
Recommendation to authorize City Attorney to submit two Stipulations with Requests for Awards for approval by the Workers' Compensation Appeals Board, and if so approved, authority to pay $8,680 and a separate authority to pay $62,576.25 for the claim of Leonard Gaeta; and $62,132.52 for Stipulations with Request for Award and $32.78 per week for a Life Pension thereafter for the claim of Sandra Thomas, in compliance with the Appeals Board Orders.
DISCUSSION
Leonard Gaeta has made a claim for injuries allegedly sustained in the course of his employment with the City of Long Beach. The Economic Development and Finance Committee on April 1, 2008, approved the submission to the Workers' Compensation Appeals Board of two Stipulations with Request for Award by the City and Leonard Gaeta, based upon 1) a 16 percent disability rating, with provision for lifetime medical care as may be necessary, and 2) a 59 percent disability rating, with provision for lifetime medical care as may be necessary.

Sandra Thomas has made a claim for injuries allegedly sustained in the course of her employment with the City of Long Beach. The Economic Development and Finance Committee on April 1, 2008, approved the submission to the Workers' Compensation Appeals Board of Stipulations with Request for Award by the City and Sandra Thomas, based upon 1) a 70 percent disability rating, and 2) a further provision for a Life Pension at the rate of $32.78 per week after the permanent disability has been paid. There would be a provision for lifetime medical care as may be necessary.
