TITLE
Recommendation to authorize City Attorney to submit Stipulations with Requests for Awards for approval by the Workers' Compensation Appeals Board, and if so approved, authority to pay $14,050 for the claim of Daniel De Guzman; $19,337.50 for the claim of Emma Henderson; $29,785 for the claim of Kris Jonkey in compliance with the Appeals Board Orders.
DISCUSSION
Daniel De Guzman has made a claim for injuries allegedly sustained in the course of his employment with the City of Long Beach. The Economic Development and Finance Committee on January 2, 2007, approved the submission to the Workers' Compensation Appeals Board of Stipulations with Request for Award by the City and Daniel De Guzman, based upon a 16 percent disability rating, with provision for lifetim.e medical care as may be necessary.
Emma Henderson has made a claim for injuries allegedly sustained in the course of her employment with the City of Long Beach. The Economic Development and Finance Committee on January 2, 2007, approved the submission to the Workers' Compensation Appeals Board of Stipulations with Request for Award by the City and Emma Henderson, based upon a 28 percent disability rating, with provision for lifetime medical care as may be necessary.
Kris Jonkey has made a claim for injuries allegedly sustained in the course of her employment with the City of Long Beach. The Economic Development and Finance Committee on January 2, 2007, approved the submission to the Workers' Compensation Appeals Board of Stipulations with Request for Award by the City and Kris Jonkey, based upon a 35 percent disability rating, with provision for lifetime medical care as may be necessary.
