TITLE
Recommendation to authorize City Attorney to submit Stipulations with Requests for Awards for approval by the Workers' Compensation Appeals Board, and if so approved, authority to pay $36,260, and separate authority to pay $10,000 to resolve vocational rehabilitation for claim of Michael Bolling; $98,095 for Stipulations with Request Award and $38.65 per week for a Life Pension thereafter for claim of Gordan Scott Jackson; and $6,475 for claim of James Schlobohm, in compliance with the Appeals Board Orders.
DISCUSSION
Michael Bolling has made a claim for injuries allegedly sustained in the course of his employment with the City of Long Beach. The Economic Development and Finance Committee on October 3, 2006 approved the submission to the Workers' Compensation Appeals Board of Stipulations with Request for award by the City and Michael Bolling, based upon 1) a 40 percent disability rating, with provision for lifetime medical care as may be necessary, and 2) a $10,000.00 buyout of vocational rehabilitation benefits.
Gordon Scott Jackson has made a claim for injuries allegedly sustained in the course of his employment with the City of Long Beach. The Economic Development and Finance Committee on October 3, 2006, approved the submission to the Workers' Compensation Appeals Board of Stipulations with Request for Award by the City and Gordon Jackson, based upon 1) a 70 percent disability rating and 2) a further provision for a Life Pension at the rate of $38.65 per week after the permanent disability has been fully paid. Finally, there would be a provision for lifetime medical care as may be necessary.
