TITLE
Recommendation to authorize City Attorney to submit Stipulations with Requests for Awards for approval by the Workers' Compensation Appeals Board, and if so approved, authority to pay $5,600 for the claim of Greg Barnhart; $10,550 for the claim of Mark German; and $8,680 for the claim of Mary Hawkins in compliance with the Appeals Board Orders.
DISCUSSION
Greg Barnhart has made a claim for injuries allegedly sustained in the course of his employment with the City of Long Beach. The Economic Development and Finance Committee on March 21, 2006, approved the submission to the Workers' Compensation Appeals Board of Stipulations with Request for Award by the City and Greg Barnhart, based upon a 7 percent disability rating, with provision for lifetime medical care as may be necessary.
Mark German has made a claim for injuries allegedly sustained in the course of his employment with the City of Long Beach. The Economic Development and Finance Committee on March 21,2006, approved the submission to the Workers' Compensation Appeals Board of Stipulations with Request for Award by the City and Mark German, based upon a 12 1/2 percent disability rating, with provision for lifetime medical care as may be necessary.
Mary Hawkins has made a claim for injuries allegedly sustained in the course of her employment with the City of Long Beach. The Economic Development and Finance Committee on March 2 1,2006, approved the submission to the Workers' Compensation Appeals Board of Stipulations with Request for Award by the City and Mary Hawkins, based upon a 16 percent disability rating, with provision for lifetime medical care as may be necessary.
