TITLE
Recommendation to authorize City Attorney to submit Stipulations with Requests for Awards for approval by the Workers' Compensation Appeals Board, and if so approved, authority to pay $12,080 for claim of Tina Cox; $8,680 for claim of Lillia Demasi; and $61,582.50 for the claim of Ky Dinn Le in compliance with the Appeals Board Orders.
DISCUSSION
Tina Cox has made a claim for injuries allegedly sustained in the course of her employment with the City of Long Beach. The Economic Development and Finance Committee on March 7, 2006, approved the submission to the Workers' Compensation Appeals Board of Stipulations with request for Award by the City and Tina Cox, based upon a 21 percent disability rating, with provision for lifetime medical care as may be necessary.
Lillia Demasi has made a claim for injuries allegedly sustained in the course of her employment with the City of Long Beach. The Economic Development and Finance Committee on March 7, 2006, approved the submission to the Workers' Compensation Appeals Board of Stipulations with Request for Award by the City and Lillia Demasi, based upon a 16 percent disability rating, with provision for lifetime medical care as may be necessary.
Ky Dim Le has made a claim for injuries allegedly sustained in the course of his employment with the City of Long Beach. The Economic Development and Finance Committee on March 7, 2006, approved the submission to the Workers' Compensation Appeals Board of Stipulations with Request for Award by the City and Ky Dinn Le, based upon a 62 percent disability rating, with provision for lifetime medical care as may be necessary.
