DISCUSSION
On April 22, 2003, the City Council accepted a gra ward in the amount of $132,962 outreach, enrollment, utilization and retention services to children (0-5 years) and families
who may be eligible for Healthy Kids, Medi-Cal, Healthy Families or other free or low-cost health care programs in the city. This agreement is funded with monies from the Healthy
Kids Program of the First 5 LA, LA County Children and Families First - Proposition 10 Commission. The initial agreement term was from July 1,2003 through June 30,2004 with annual one-year renewals through June 30,2008.
from the County of Los Angeles Department of Heath Services (County) to provide The County has presented an amendment to the agreement adding language which allows the County to terminate the agreement with or without cause with a 30-day notice, and eliminates the target numbers in the scope of work that the County deemed not measurable. Additionally, the DHHS works with subcontractors to carry out the goals of this grant. Of the $132,962 in funds from the County annually, the DHHS utilizes $85,000 to sustain outreach, enrollment, and case-management subcontracts with the following collaborative
community-based agencies that were selected based on their ability to serve special population groups:
To preserve the continuity of the grant's services, the DHHS requests the ability to automatically renew these subcontracts annually for the duration of the grant period provided that the County renews the grant.
