TITLE
Recommendation to authorize City Attorney to submit Stipulations with Request for Award for approval by the Workers' Compensation Appeals Board, and if so approved, authority to pay $181,777.50 for Stipulations with Request for Award, and also 2) to pay $193.27 per week for a Life Pension thereafter, subject to cost of living adjustments for the claim of Betty Jordan-Henry; and authority to pay 1) $156,562.50 for Stipulations with Request for Award with a life pension thereafter of $119.83 per week and 2) separate authority to pay $3,300 for the claim of Gregory Keyes, in compliance with the Appeals Board Orders.
DISCUSSION
Betty Jordan-Henry has made a claim for injuries allegedly sustained in the course of her employment with the City of Long Beach. The Economic Development and Finance Committee on October 1, 2013, approved the submission to the Workers' Compensation Appeals Board of Stipulations with Request for Award by the City and Betty Jordan-Henry, based upon (1) an 85 percent disability rating and (2) a further provision for a Life Pension at the rate of $193.27 per week, subject to cost of living adjustments, after the permanent disability has been fully paid. Finally, there would be a provision for lifetime medical care as may be necessary;
and
Gregory Keyes has made a claim for injuries allegedly sustained in the course of his employment with the City of Long Beach. The Economic Development and Finance Committee on October 1, 2013, approved the submission to the Workers' Compensation Appeals Board of two Stipulations with Request for Award by the City and Gregory Keyes, based upon 1) a 90 percent disability rating with further provision for a life pension at the rate of $119.83 per week after the permanent disability has been paid, with provision for lifetime medical care as may be necessary, and 2) a 5 percent disability rating, with provision for lifetime medical care as may be necessary.
