TITLE

DISCUSSION
Since 1995, the California State Department of Health Care Services (Medi-Cal) has allocated funds to the City of Long Beach, Department of Health and Human Services (Health Department) to administer the Child Health and Disability Prevention (CHDP) Program. The State of California (State) has allocated $593,892 to the Health Department to carry out CHDP Program requirements as set forth in Health and Safety Code, Article 6, Section 124025 et seq. and Article 4.7, Section 14148.3 of the Welfare and Institutions Code for the period of July 1, 2012 through June 30, 2013.

Under State requirements, the Health Department provides the required scope of work activities, which include certifying new local CHDP providers, conducting quality assurance activities with existing CHDP providers, conducting provider education sessions, reviewing CHDP health assessment results and providing short-term case management for children with identified medical conditions, referring children and families to resources for health care, and maintaining an Interagency Agreement with the Los Angeles County Departments of Public Social Services, Children and Family Services, and Probation for coordination of CHDP Services.

Health Department CHDP Public Health Nurses provide administrative oversight to over forty CHDP medical providers located throughout the City of Long Beach. These providers process over 4,000 CHDP exams per month. Health Department staff reviews, monitors and updates over 3,500 CHDP health assessments each month.
