TITLE

DISCUSSION
Richard Montoya has made a claim for injuries allegedly sustained in the course of his employment with the City of Long Beach.  The Economic Development and Finance Committee on August 7, 2012, approved the submission to the Workers' Compensation Appeals Board of Stipulations with Request for Award by the City and Richard Montoya, based upon a 69 percent disability rating, with provision for lifetime medical care as may be necessary.

Darren Rockett, by and through his Widow, Jeanne Rockett and their children, Alyse J. Rockett, Sydney Rockett and Daryn Rockett have made a claim for death benefits allegedly caused by his employment with the City of Long Beach.  The Economic Development and Finance Committee on August 7, 2012, approved the submission to the Workers' Compensation Appeals Board of Stipulations with Request for Award for death benefits by the City and Darren Rockett on behalf of his widow and dependents.
