Title: Recommendation to authorize City Attorney to submit two Stipulations with Request for Award for approval by the Workers' Compensation Appeals Board, and if so approved, authority to pay (1) $73,565.91, and (2) separate authority to pay $19,674.04. TITLE

DISCUSSION
Jan Andriese has made a claim for injuries allegedly sustained in the course of his employment with the City of Long Beach. The Economic Development and Finance Committee on March 24, 2015, approved the submission to the Workers' Compensation Appeals Board of Stipulations with Request for Award by the City and Jan Andriese, based upon a 1) a 51 percent disability rating, with provision for lifetime medical care as may be necessary, and 2) a 20 percent disability rating, with provision for lifetime medical care as may be necessary.
 
Philip Golia has made a claim for injuries allegedly sustained in the course of his employment with the City of Long Beach. The Economic Development and Finance Committee on March 24, 2015, approved the submission to the Workers' Compensation Appeals Board of Stipulations with Request for Award by the City and Philip Golia, based upon a 62 percent disability rating, with provision for lifetime medical care as may be necessary.
