Recommendation to authorize City Manager, or designee, to execute all necessary documents and amendments between the State of California Department of Health Care Services and the City of Long Beach for the continued provision of Targeted Case Management Services to eligible Medi-Cal beneficiaries for a period of five years, July 1, 2013 through June 30, 2018, for an estimated annual revenue of $185,000. DISCUSSION
Since 1997, the City of Long Beach Department of Health and Human Services (Health Department), has been providing Targeted Case Management (TCM) services to eligible Medi-Cal beneficiaries. Under this agreement with the State of California Department of Health Care Services (State), Health Department staff assist eligible clients in accessing critical medical services.
This year, over 800 individuals received TCM case management services from the Health Department. The beneficiaries are mostly children, and medically fragile individuals. TCM provides critical case management services to individuals with significant health needs.
On September 18, 2012, the City Council approved the most recent TCM Provider Participation Agreement (PPA) which extended the contract for one year, ending on June 30, 2013. The proposed agreement will allow the Health Department to continue to claim reimbursement for the provision of TCM services. The term of the agreement is July 1, 2013 through June 30, 2018.

TIMING CONSIDERATIONS
City Council action on this matter is requested on June 11, 2013 to ensure timely billing and revenue capture.
FISCAL IMPACT
