TITLE
Recommendation to authorize City Attorney to submit Stipulations with Request for Award for approval by the Workers' Compensation Appeals Board, and if so approved, authority to pay 1) $46,230, and 2) separate authority to pay $24,552.50, and 3) separate authority to pay $147,217.50 with a life pension thereafter of $131.42 per week for the claim of Phillip Coombs; authority to pay $108,822.50 for the claim of James La Barba; and authority to pay $69,747.50 for the claim of Richard Miller, in compliance with the Appeals Board Orders.
DISCUSSION
Phillip Coombs has made a claim for injuries allegedly sustained in the course of his employment with the City of Long Beach. The Economic Development and Finance Committee on June 20, 2017, approved the submission to the Workers’ Compensation Appeals Board of three Stipulations with Request for Award by the City and Phillip Coombs, based upon 1) a 40 percent disability rating, with provision for lifetime medical care as may be necessary, and 2) a 26 percent disability rating, with provision for lifetime medical care as may be necessary, and 3) a 77 percent disability rating with further provision for a life pension at the rate of $131.42 per week after the permanent disability has been paid, with provision for lifetime medical care as may be necessary.
James La Barba has made a claim for injuries allegedly sustained in the course of his employment with the City of Long Beach. The Economic Development and Finance Committee on June 20, 2017, approved the submission to the Workers’ Compensation Appeals Board of Stipulations with Request for Award by the City and James La Barba, based upon a 63 percent disability rating with provision for lifetime medical care as may be necessary.
