TITLE

DISCUSSION

George Ayala has made a claim for injuries allegedly sustained in the course of his employment with the City of Long Beach. The Economic Development and Finance Committee on October 17, 2017, approved the submission to the Workers' Compensation Appeals Board of three Stipulations with Request for Award by the City and George Ayala, based upon 1) a 34 percent disability rating with provision for lifetime medical care as may be necessary 2) an 18 percent disability rating with provision for lifetime medical care as may be necessary, and 3) a 20 percent disability rating with provision for lifetime medical care as may be necessary.

SUGGESTED ACTION

Approve recommendation.
