TITLE
Recommendation to authorize City Attorney to submit Stipulations with Requests for Awards for approval by the Workers' Compensation Appeals Board, and if so approved, authority to pay $116,725 and $73.44 per week for a life pension for the claim of Lee Anthony; $8,040 for the claim of Theresa Maceyka; and $114,665 for the claim of Roland Rose, in compliance with Appeals Board Orders.
 
DISCUSSION
Lee Anthony has made a claim for injuries allegedly sustained in the course of his employment with the City of Long Beach. The Economic Development and Finance Committee on March 13, 2007, approved the submission to the Workers' Compensation Appeals Board of Stipulations with Request for Award by the City and Lee Anthony, based upon 1) a 79 percent disability, and 2) a further provision for a Life Pension at the rate of $73.44 per week after the permanent disability has been fully paid.
Finally, there would be a provision for lifetime medical care as may be necessary.
 
Theresa Maceyka has made a claim for injuries allegedly sustained in the course of her employment with the City of Long Beach. The Economic Development and Finance Committee on March 13, 2007, approved the submission to the Workers' Compensation Appeals Board of Stipulatoins with Request for Award by the City and Theresa Maceyka, based upon a 15 percent disability rating, with provision for lifetime medical care as may be necessary.
 
Roland Rose has made a claim for injuries allegedly sustained in the course of his employment with the City of Long Beach. The Economic Development and Finance Committee on March 13, 2007, approved the submission to the Workers' Compensation Appeals Board of Stipulations with Request for Award by the City and Roland Rose, based upon a 78 percent disability rating, with provision for lifetime medical care as may be necessary.
