TITLE
Recommendation to authorize City Attorney to submit Stipulations with Requests for Awards for approval by the Workers' Compensation Appeals Board, and if so approved, authority to pay $11,174 for the claim of Maria Alarcon; $5,720 for claim of Erik Burks; $114,600.55 with further provision for a life pension, and $10,000 for vocational rehabilitation benefits for the claim of Brian Grogan; $16,277.50 and a separate authority to pay $9,960 for the claim of Hugh Thomas McNicholl; $3,960 and a separate authority to pay $9,295 for the claim of Richard Montoya; and $12,210 for the claim of Rudy Romero, in compliance with the Appeals Board Orders.
DISCUSSION
Maria Alarcon has made a claim for injuries allegedly sustained in the course of her employment with the City of Long Beach. The Economic Development and Finance Committee on December 5, 2006, approved the submission to the Workers' Compensation Appeals Board of Stipulations with Request for Award by the City and Maria Alarcon, based upon a 21 percent disability rating, with provision for lifetime medical care as may be necessary.
Erik Burks has made a claim for injuries allegedly sustained in the course of his employment with the City of Long Beach. The Economic Development and Finance Committee on November 21, 2006, approved the submission to the Workers' Compensation Appeals Board of Stipulations with Request for Award by the City and Erik Burks, based upon a 6 % percent disability rating, with provision for lifetime medical care as may be necessary.
Brian Grogan has made a claim for injuries allegedly sustained in the course of his employment with the City of Long Beach.
