TITLE

DISCUSSION

Brenda Relph, by and through her widow, Tracey Henk, has made a claim for death benefits allegedly caused by her employment with the City of Long Beach.  The Economic Development and Finance Committee, on November 5, 2013, approved the submission to the Workers' Compensation Appeals Board of Stipulations with Request for Award for Death Benefits by the City and Brenda Relph on behalf of her widow.

Approve recommendation.
