TITLE
Recommendation to authorize City Manager to execute all necessary agreements with the County of Los Angeles Department of Public Health in the amount of $130,000 annually for five years to provide Community Prevention and Wellness Services under the Community Transformation Grant (CTG) program, administer and execute all necessary documents to accept and expend all grant funds in connection herewith, take all other actions necessary to implement the use of such funds; and increase appropriations in the Health Fund
(SR 130) in the Department of Health and Human Services (HE) by $130,000 for the period of June 2012 to September 2016. (Citywide)
DISCUSSION
In September 2011, the Los Angeles County Department of Public Health was awarded a federal grant from the Prevention and Public Health Fund: Community Transformation Grant (CTG) administered by the Centers for Disease Control and Prevention (CDC) to support intensive approaches to reduce risk factors responsible for the leading causes of death and disability, and to prevent and control chronic diseases.
The County has offered funding to the City of Long Beach Department of Health and Human Services (Health Department) to manage a diabetes prevention program for Spanish-speaking community members throughout Long Beach. The Health Department will receive an allocation of up to $130,000 annually for five years to continue its efforts in the area of diabetes prevention and management through the current Latino Diabetes Prevention and Management Program.

TIMING CONSIDERATIONS
City Council action is requested on July 3, 2012, as the agreement must be executed with Los Angeles County by July 20, 2012.
