TITLE

DISCUSSION
Since 1974, the City of Long Beach (City) has provided services to improve family wellness at the following four facilities:

· Central Facilities Center, 1133 Rhea Street
· North Facilities Center, 6335 Myrtle Avenue
· West Facilities Center, 2125 Santa Fe Avenue
· Miller Family Health Education Center, 3820 Cherry Avenue

In addition to family wellness services, community-based services have been provided by a variety of local non-profit agencies that occupy office space at these sites through a Facility Use Permit. Agencies utilizing space at these various facilities are required to be not for profit agencies and must address a critical health services need at the local level. Space is provided on a first-come, first-served basis, based upon availability and Health and Human Services Department priorities. All agencies must meet City insurance requirements.

The State of California Department of Public Health has requested to enter into a Facility Use Permit for space located at the Miller Family Health Education Center (Center). The State of California Department of Public Health provides the City with direct assistance in Sexually Transmitted Disease (STD) control and prevention surge capacity, specifically around syphilis case management, investigations, interviews, and contact elicitations as part of partner services on behalf of the City.
