TITLE

Recommendation to authorize City Attorney to submit claim for death benefits for approval by the Workers’ Compensation Appeals Board, and if so approved, authority to pay $300,000, for the claim of Kimberly Maddox, in compliance with the Appeals Board Order.

 

DISCUSSION

Kimberly Maddox, by and through her Widow, Maria Macias, and their children, Anthony Scott Macias and Jasmine Celeste Macias have made a claim for death benefits allegedly caused by her employment with the City of Long Beach. The Economic Development and Finance Committee on April 18, 2017, approved the submission to the Workers’ Compensation Appeals Board of Stipulations with Request for Award for death benefits by the City and Kimberly Maddox on behalf of her widow and dependents.

Approve recommendation.
