TITLE
Recommendation to authorize City Attorney to submit Stipulations with Requests for Awards for approval by the Workers' Compensation Appeals Board, and if so approved, authority to pay (1) $36,260 for permanent disability rating, and (2) $5,000 to resolve vocational rehabilitation benefits for claim of Randy Butler; two Stipulations with Requests for Awards in the amount of (1) $65,622.50 and (2) $30,940 for claim of David Edwards; (1) $58,800 and (2) $5,000 to resolve vocational rehabilitation benefits for claim of Robbie Hill-Morrison; $7,816.25 for claim of Michael Olson; and $13,310 for claim of Alberto Vargas, in compliance with Appeals Board Order.
DISCUSSION
Randy Butler has made a claim for injures allegedly sustained in the course of his employment with the City of Long Beach. The Economic Development and Finance Committee on September 5, 2006, approved the submission to the Workers' Compensation Appeals Board of Stipulations with Request for Award by the City and Randy Butler, based upon (1) a 40 percent disability rating, with
provision for lifetime medical care as may be necessar, and (2) a $5 000.00 buy out of vocational rehabilitation benefits.
SUGGESTED ACTION
Approve recommendation.
