TITLE
Recommendation to authorize City Attorney to submit Stipulations with Request for Award and a Compromise and Release for approval by the Workers' Compensation Appeals Board, and if so approved, authority to pay $17,713.50 for Stipulations with Request for Award; and a separate authority to pay $4,999 for a Compromise and Release, in compliance with the Appeals Board Order for claim of David Rosa.
DISCUSSION
David Rosa has made a claim for injuries allegedly sustained in the course of his employment with the City of Long Beach. The Economic Development and Finance Committee on November 21, 2006, approved the submission to the Workers' Compensation Appeals Board of 1) Stipulations with Request for Award by the City and David Rosa, based upon a 24 percent disability rating, with provision for lifetime medical care as may be necessary and 2) a Compromise & Release in the amount of $4,999.00.
