DISCUSSION
William Ardizzone has made a claim for injuries allegedly sustained in the course of his employment with the City of Long Beach. The Economic Development and Finance Committee on January 7, 2014, approved the submission to the Workers' Compensation Appeals Board of Stipulations with Request for Award by the City and William Ardizzone, based upon (1) a 68 percent disability rating, with provision for lifetime medical care as may be necessary, and (2) a 4 percent disability rating with provision for lifetime medical care as may be necessary;
 
Louie Balestrieri has made a claim for injuries allegedly sustained in the course of his employment with the City of Long Beach. The Economic Development and Finance Committee, on January 7, 2014, approved the submission to the Workers' Compensation Appeals Board of (1) Stipulations with Request for Award by the City and Louie Balestrieri, based upon a 60 percent disability rating, with provision for lifetime medical care as may be necessary and separate authority to pay (2) $15,000.00 in compliance with the Appeals Board Order;
 
Larry Huffman has made a claim for injuries allegedly sustained in the course of his employment with the City of Long Beach. The Economic Development and Finance Committee on January 7, 2014, approved the submission to the Workers' Compensation Appeals Board of Stipulations with Request for Awards by the City and Larry Huffman, based upon (1) a 42 percent disability rating, with provision for lifetime medical care as may be necessary, and (2) a 4 percent disability rating, with provision for lifetime medical care as may be necessary, and (3) an 8 percent disability rating, with provision for lifetime medical care as may be necessary, and (4) a 12 percent disability rating, with provision for lifetime medical care as may be necessary;
 
Donald Hulse has made a claim for injuries allegedly sustained in the course of his employment with the City of Long Beach. The Economic Development and Finance Committee, on January 7, 2014, approved the submission to the Workers' Compensation Appeals Board of Stipulations with Request for Award by the City and Donald Hulse, based upon (1) a 74% disability rating and (2) a further provision for a Life Pension at the rate of $108.23, subject to Cost of Living Adjustments, after the permanent disability has been fully paid. Finally, there would be a provision for lifetime medical care as may be necessary;
 
James Kovacs has made a claim for injuries allegedly sustained in the course of his employment with the City of Long Beach. The Economic Development and Finance Committee on January 7, 2014, approved the submission to the Workers' Compensation Appeals Board of Stipulations with Request for Award by the City and James Kovacs, based upon a 55 percent disability rating, with provision for lifetime medical care as may be necessary; and
 
Joseph Smith has made a claim for injuries allegedly sustained in the course of his employment with the City of Long Beach. The Economic Development and Finance Committee on January 7, 2014, approved the submission to the Workers' Compensation Appeals Board of Stipulations with Request for Award by the City and Joseph Smith, based upon a 69 percent disability rating, with provision for lifetime medical care as may be necessary.
