Mr. Philip Bernstein has made a claim for injuries allegedly sustained in the course of his employment with the City of Long Beach.

 

The Economic Development and Finance Committee, on April 20, 2004 approved the submission to the Workers' Compensation Appeals Board of Stipulations with Request for Award by the City and Mr. Bernstein, based upon a 14 percent permanent disability rating, with provision for lifetime medical care as may be necessary.

 

We, therefore, request authority to submit said Stipulations with Request for Award for approval by the Workers' Compensation Appeals Board, and if so approved, authority to pay $6,475.00 in compliance with the Appeals Board order.

 

AND

 

Mr. Richard Moreno has made a claim for injuries allegedly sustained in the course of his employment with the City of Long Beach.

 

The Economic Development and Finance Committee, on April 20, 2004, approved the submission to the Workers' Compensation Appeals Board of Stipulations with Request for Award by the City and Mr. Moreno, based upon a 31 percent permanent disability rating, with provision for lifetime medical care as may be necessary.

 

We, therefore, request authority to submit said Stipulations with Request for Award for approval by the Workers' Compensation Appeals Board, and if so approved, authority to pay $22,610.00 in compliance with the Appeals Board order.

 

AND

 

Ms. Adela Rose has made a claim for injuries allegedly sustained in the course of her employment with the City of Long Beach.

 

The Economic Development and Finance Committee, on April 20, 2004 approved the submission to the Workers' Compensation Appeals Board of Stipulations with Request for Award by the City and Ms. Rose, based upon a 15 percent permanent disability rating, with provision for lifetime medical care as may be necessary.

 

We, therefore, request authority to submit said Stipulations with Request for Award for approval by the Workers' Compensation Appeals Board, and if so approved, authority to pay $8,040.00 in compliance with the Appeals Board order.

 

AND

 

Johnny G. Silva has made a claim for injuries allegedly sustained in the course of his employment with the City of Long Beach.

 

The Economic Development and Finance Committee, on April 20, 2004, approved the submission to the Workers' Compensation Appeals Board of Stipulations with Request for Award by the City and Johnny G. Silva, based upon a 27 percent permanent disability rating, with provision for lifetime medical care as may be necessary.

 

We, therefore, request authority to submit said Stipulations with Request for Award for approval by the Workers' Compensation Appeals Board, and if so approved, authority to pay $18,317.50 in compliance with the Appeals Board order.

 

SUGGESTED ACTION

Authorize City Attorney to submit for approval by Workers' Compensation Appeals Board, and if so approved, authority to pay $6,475 in compliance with Appeals Board order for claim of Philip Bernstein; $22,610 for claim of Richard Moreno; $8,040 for claim of Adela Rose; and $18,317.50 for claim of Johnny Silva.
