DISCUSSION

Community Partners was established in 1992 to offer expert guidance, essential services, and support to grow creative solutions to community challenges. Community Partners has awarded the City of Long Beach a $200,000 capacity building grant for an initial period of two years.  The grant is intended to support the Health and Human Services Department’s (Health Department) participation in the California Accountable Communities for Health Initiative (CACHI). 

 

Through CACHI, the Health Department will plan and implement a model system that links health care systems, providers and health plans to public health, and community and social services organizations serving diverse communities across California.  Grounded in an innovative vision to reform the health care system so that it is more responsive to people’s needs and environments, this model system would set a precedent and pave the way for developing health care systems that are more expansive, connected, and prevention oriented.

 

Since June 1, 2017, the Health Department has used CACHI funds to develop an Accountable Community of Health (ACH) for children.  The funding from Community Partners will enable the Health Department to continue to work in establishing an ACH model for All Children Thrive, which creates a community environment that supports positive development in children ages zero to eight.  ACH models align medical providers, non-profit organizations, communities, and government partners to support the health and wellness of the community. 

 

This matter was reviewed by Deputy City Attorney Taylor M. Anderson on September 25, 2019 and by Budget Management Officer Rhutu Amin Gharib on October 7, 2019.

 

TIMING CONSIDERATIONS

City Council action is requested on October 22, 2019, to continue the ACH program without interruption.

 

FISCAL IMPACT

The City will receive funding in the amount of $200,000 for the period of September 1, 2019 through August 31, 2021.  An appropriation increase in the amount of $200,000 is requested in the Health Fund Group in the Health and Human Services Department, offset by grant revenue.  Subsequent year’s appropriations will be requested through the annual budget process.  There is a match requirement of $50,000 via in-kind services and $50,000 via other existing community grants. This recommendation has no staffing impact beyond the normal budgeted scope of duties and is consistent with existing City Council priorities.  There is no local job impact associated with this recommendation.
