DISCUSSION
Diabetes is a chronic disease that can cause serious health complications, including blindness, kidney failure, and heart disease.  Diabetes is among the top five causes of death in the City of Long Beach.  Mostly impacting minority groups, the percentage of adults diagnosed with diabetes, sugar diabetes or borderline/pre-diabetes in Long Beach by zip code ranges from 15 to 24 percent (Community Health Assessment, 2013).  The 90813 zip code has the highest rate at 24.3 percent of the adult population receiving either a diabetes or pre-diabetes diagnosis.
 
The Colorado Foundation for Public Health and the Environment has awarded the City of Long Beach $15,000 for the Department of Health and Human Services' (Health Department) Diabetes Program to develop a community-informed, patient-centered action plan and grant proposal to improve health outcomes for pre-diabetes (reverse or halt advancement) and reduce costs through prevention and management.
 
The Familias Unidas Pre-Diabetes Diagnosis and Treatment Project will be based on a partnership between the Health Department, the National Council of La Raza/California State University Long Beach Center for Latino Community Health, and St. Mary's Medical Center.  This partnership will explore how best to address pre-diabetes diagnosis and treatment to reduce or delay the onset of diabetes among low-income Latino communities.
 
This matter was reviewed by Deputy City Attorney Linda Vu on May 12, 2015 and by Budget Management Officer Victoria Bell on May 15, 2015.
 
TIMING CONSIDERATIONS
City Council action on this item is requested on June 9, 2015 in order to accept funding and begin program operations to meet required objectives.
 
FISCAL IMPACT
The total project cost is estimated at $17,700.  Of this amount, the Department of Health and Human Services will provide an in-kind contribution of approximately $2,700 for facility set-up, audio visual, tech support, web-hosting, computers and interpretation services throughout the project period of May 1, 2015 through February 1, 2016.  The remaining cost of $15,000 will be funded by the grant agreement.  Sufficient funds are budgeted in the Health Fund (SR 130) in the Department of Health and Human Services (HE).  There is no local job impact associated with this action.
